• The Victorian Secondary School Vaccine Program consent cards are distributed to students and parent/guardians by schools.

  • The cards contain important information regarding the vaccines offered to secondary school students, including possible side effects.

  • Parents/guardians of Years 7 students must complete and sign PART A of the Year 7 consent card booklet, regardless of whether or not the student is to receive vaccines at school.

  • Parents/guardians of Years 10 students must complete and sign the Meningococcal ACWY consent card booklet, regardless of whether or not the student is to receive vaccines at school.

  • Consent forms and disease information are available in languages other than English.
